Register

As part of our contribution to the ‘Everyone a GP in Delft’ project, which has been set up to ensure continuity of GP care, we are offering (new) residents who do not yet have a GP the chance to register with our practice.

You can register below from 1 April 2025. Registrations will be assessed based on the following criteria:

– The patient does not currently have a GP in Delft and has never had a GP in Delft. If a patient deregisters with their current GP in an attempt to meet these criteria, we will not register them at our practice, and

– The patient must reside in Delft in one of the following postcodes: 2611 (up to and including Vrijenbansekade), 2628, 2624, 2627 (up to and including Deltaweg) and 2613 (up to and including Alexander Fleminglaan).

Drop-in or temporary patients are excluded from this project. This chance to register at our practice is only being offered in the regular registration category.

Depending on how busy the practice is with existing patients and the number of new registrations received, the actual process of registering new patients may take several weeks.

You can register via this form. Please note that this is a registration form only. Filling it in does not mean that you are automatically registered at the practice. After we have reviewed your details, you will be informed whether you meet the criteria and if your registration has been accepted. It is possible that you may be invited for an introductory appointment to assess your expectations of GP care before you can be registered.
